Born in 2020, the technology-based and research-based company AI LABS GROUP SL has been consolidated in the health sector thanks to Legit.Health, a revolutionary medical device that is transforming dermatology. Backed by CDTI Innovation and the European funds of the Recovery and Resilience Mechanism (MRR), this “clinical artificial intelligence” tool is able to analyze images taken with a mobile phone to support diagnosis, measure severity and monitor hundreds of skin conditions, transforming the interaction between doctors and patients from the root.

The creation of the company arose from a clear need detected in the healthcare system that traditional systems could not solve: the huge waiting lists and the widespread difficulty in accessing a dermatology appointment. "Founding the company was an act of conviction. What began as a concern became a project, understanding that the shortage of specialists is a global problem and that in many autonomous communities appointments can be extended for months," explains Andy Aguilar Robles, CEO of the firm.

The origin of the project Legit.Health is also linked to his own experience, since as a chronic patient of dermatology he experienced in the first person the lack of objectivity in the monitoring of his pathology. With this challenge in mind, the company was born combining five complementary perspectives: that of the patient, that of the artificial intelligence scientist, that of the product technologist, that of the engineer and that of the medical dermatologist. "That diversity, together with a shared mission, is what allowed us to move from an idea to a real solution, scalable and with clinical rigor from day one," says Aguilar Robles.


Visual innovation for precision diagnostics

The success of Legit.Health in the field of dermatology is based on its purely visual character, which allows remote diagnosis. “Where previously there was subjective interpretation, artificial intelligence can provide an objective and reproducible measurement,” he explains. In addition, the impact in this area is vital: “Pathologies such as melanoma critically depend on early detection, and each month of delay significantly reduces survival,” the CEO adds, stressing that the goal of the tool is not to replace the doctor, but to help him, so that serious cases are detected and dealt with in time.

Legit.Health acts as an intelligent assistant that puts AI at the service of the professional to speed up clinical decisions based on a mobile photograph. By capturing the image of the injury, the system deploys a triple key function in the day to day care. The software starts the process with the identification and diagnostic orientation of signs in more than 300 skin diseases, and then executes a continuous follow-up through the objective and reproducible measurement of the severity of each lesion. This automated analysis ends up facilitating the prioritization of the most urgent cases, thus optimizing the management of waiting lists.

The great competitive advantage of this software compared to other market solutions lies in its technical and regulatory rigor. "We don't just classify an image; we quantify severity and evolution, which is what a doctor really needs. In addition, we have built the product from the beginning as a medical device with the rigor and clinical validation required by the European regulatory framework, the CE marking," says Andy Aguilar.


Technical challenges and an invisible integration

The development of highly complex medical algorithms entails significant infrastructure challenges. The first of these is access to reliable data, requiring a massive volume of high-quality clinical images, correctly labeled by specialists and representative of all the diversity of skin tones to avoid bias. To this is added the need for a large computing capacity and the requirements of the regulatory environment, which obliges to document and scientifically validate each decision of the algorithm. “It’s a very high bar, but it’s exactly the bar that medicine deserves,” he says.

Despite its internal complexity, day-to-day interaction is extremely simple thanks to its flexible architecture. More than a standalone application, Legit.Health works as a technology that integrates into the tools that hospitals and telemedicine platforms already use, such as electronic medical records. “We don’t ask anyone to change the way they work: we add an objective measuring layer within their usual flow,” he explains.

Thus, when the image of the injury is captured, the AI analyzes it in seconds and the doctor receives the case already contextualized. "This time saving allows prioritization and filtering so that urgent cases are dealt with before and the mild ones do not fill the agenda, also allowing remote monitoring that avoids unnecessary displacements. In the end, everyone wins: the system gains efficiency and the patient gains peace of mind and speed,” he adds.

 

Clinical evidence and the strategic endorsement of the CDTI

To advance its roadmap towards large-scale implementation and ambitious international expansion, public administration support has made a difference. In this sense, the CEO describes as a "decisive impulse" the support of CDTI Innovation through its grants Seal of Excellence, a line that finances R&D projects of Spanish SMEs recognized with this distinction of quality by the European Innovation Council within the Horizon Europe program. "In material terms, developing science-based technology and a medical device requires a sustained investment in research, and this support has allowed us to undertake development and validation phases that would otherwise have been much slower," says Aguilar Robles.

Likewise, the CEO highlights the intangible value that the seal of a reference institution such as the CDTI brings in terms of credibility, validating the solidity of the proposal before clients and strategic partners: “For a technology-based company like ours, that recognition opens doors, attracts talent and strengthens the trust of the professionals and partners we work with. We are very grateful for that accompaniment.”

Looking to the future, the AI LABS GROUP strategy is oriented towards the medium-term consolidation of the implementation of Legit.Health in more hospitals and public and private health systems. The company plans to expand the clinical scope of the technology, continue to reinforce the scientific evidence of its functionalities and make objective measurement of the skin such a natural and habitual process in consultations as measuring blood pressure.

In the long term, the goals focus on positioning Legit.Health as a global reference in artificial intelligence applied to dermatology. The firm aspires to demonstrate its full international potential from its headquarters in the Basque Country. “We would like to play the role of those who demonstrate that artificial intelligence in health can be done with the utmost rigor, in a responsible, safe and people-centered way,” he concludes.
